Enrollment Process

Joining Building Bloch’s Pathways is a collaborative, step-by-step process designed to ensure services are the right fit and fully aligned with Regional Center and Self-Determination Program (SDP) requirements.

  1. Reach Out to Us

    Contact our team directly by phone or email to express interest and learn more about our services.

  2. Schedule an Intake Consultation (Zoom)

    We schedule a Zoom intake meeting to:

    • Learn about the individual’s goals, strengths, and support needs
    • Discuss eligibility, service options, and staffing models
    • Answer initial family questions
  3. Initial Assessment & Program Fit Review

    Our team reviews information shared during the intake to determine:

    • Program fit
    • Support ratios
    • Scheduling availability
    • Alignment with community-based goals
  4. Coordinate With Your Service Coordinator or Independent Facilitator

    With family consent, we collaborate directly with the Regional Center Service Coordinator and/or Independent Facilitator (IF) to:

    • Confirm services
    • Review IPP goals and safety needs
    • Ensure alignment with SDP guidelines
  5. Add Services to the SDP Spending Plan

    Our services are added to the individual’s SDP Spending Plan, including:

    • Approved service codes
    • Support levels
    • Authorized hours

    (Services must be approved before scheduling can begin.)

  6. Select and Contact a Financial Management Service (FMS)

    Families choose an FMS to manage payroll, invoicing, and payments.

    We support coordination as needed to ensure smooth onboarding.

  7. Complete FMS & Provider Onboarding

    Once the Spending Plan is approved:

    • Required onboarding documents are completed
    • Provider setup is finalized with the FMS
    • Start dates are confirmed
  8. Schedule Services & Begin Programming

    After all approvals and onboarding are complete, we:

    • Finalize the schedule
    • Assign staff
    • Begin services
  9. Our team remains in close communication to ensure a smooth and supported start.

We continue collaborating with families, Regional Centers, and IFs to adjust services as goals evolve and needs change.
